What Root Cause Analysis Is and Why It Drives QualityUntitledA Structured Route from Symptom to Solution

Root Cause Analysis (RCA) is a structured problem-solving discipline that identifies the fundamental causes of defects, failures and incidents so they can be eliminated for good, rather than patched at the symptom level. Root cause analysis training in Ireland gives you a practical, tool-based method to investigate any problem with rigour and confidence. The course moves you through the six-step RCA investigation cycle and a complete toolkit: the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in an RCA context, Is/Is Not analysis and the 8D methodology. You learn when to apply each tool based on how complex or safety-critical a problem is, then how to develop and verify corrective actions that hold. RCA is the backbone of ISO 9001:2015 corrective action (Clause 10.2), the Six Sigma DMAIC Analyse phase and Lean problem-solving. That makes it directly relevant to quality engineers, operations managers, health and safety leads and improvement practitioners across Ireland's life sciences, manufacturing and technology employers. Build the skill with Invensis Learning and start preventing recurrence from day one.

Why RCA Skills Are in Demand Across Irish Industry

    CAPA Under Regulatory Scrutiny

Irish medtech and pharma sites must satisfy FDA, EU MDR and ISO 13485 CAPA expectations. Weak root cause investigation is a common inspection finding, and structured RCA is the fix.

RCA builds audit-ready corrective action skills

    Recurring Defects Erode Margin

Manufacturers lose money to repeat defects, complaints and rework when teams treat symptoms. RCA identifies the systemic cause so the same problem does not return next quarter.

RCA prevents recurrence at the root

    Problem-Solving Skills Are Scarce

As Irish life sciences races toward 56,000 medtech jobs by 2028, employers struggle to find people who can run rigorous investigations. Structured RCA capability is in short supply.

    RCA makes practitioners stand out

    Inconsistent Investigation Methods

When each investigator uses a different approach, corrective actions vary in quality and are hard to audit. A common RCA toolkit standardises how problems are analysed and documented.

RCA creates a shared, repeatable method

    8D Demanded Across Supply Chains

Automotive, electronics and OEM supply chains expect 8D reports as standard responses to quality escapes. Teams need the discipline to complete containment, root cause and corrective action.

RCA covers full 8D problem solving

    Safety Incidents Need Systemic Answers

EHS teams in energy, utilities and manufacturing must show incidents were investigated to root cause, not just closed. Fault Tree Analysis and event mapping deliver that rigour.

    RCA builds structured incident analysis

RCA is more than a technique. It is the skill that stops problems coming back and keeps regulated operations audit-ready. Join Irish quality and operations professionals who fix causes, not symptoms. Start your journey today.

The RCA Toolkit You Will MasterSkills You Can Apply the Very Next Day

This one-day programme builds a complete, practical problem-solving toolkit. By the end you can define a problem, select and apply the right RCA tool, and develop corrective actions that prevent recurrence across any process or sector.

Define Problems Precisely with Is/Is Not

Write clear, factual problem statements and use Is/Is Not analysis to set the boundary of an issue. You learn to separate observation from assumption so your investigation starts on solid ground, the single biggest driver of a successful RCA.

Drill to the Root Using the 5 Whys

Apply the 5 Whys to trace a symptom back to its systemic cause without stopping at blame. You practise recognising a true root cause, verifying the chain, and knowing when the technique fits a simple, single-cause problem and when it does not.

Map Every Cause with Fishbone Diagrams

Facilitate Fishbone (Ishikawa) workshops using the 6M categories to surface all potential causes of a complex problem. You learn to organise team input, drill each branch, and validate the most likely causes with evidence before acting.

Analyse Complex Failures with FTA and FMEA

Use Fault Tree Analysis to model how a top event can occur through AND and OR logic, and apply FMEA to prioritise corrective actions by risk. These tools bring rigour to safety-critical and multi-cause investigations.

    Run Structured Investigations with 8D

Work through all eight disciplines of the 8D methodology, from team formation and interim containment to root cause, permanent corrective action and prevention of recurrence. It is the standard many OEMs and regulated employers expect.

    Build and Verify Corrective Actions

Design corrective actions that address the verified root cause, then confirm they work through verification and validation. You connect the method to ISO 9001:2015 Clause 10.2 and CAPA so your investigations hold up in audit.

Frequently Asked QuestionsHere's everything you may ask...

Root cause analysis (RCA) training is a practical skills course that teaches you to investigate problems and eliminate their underlying causes. It suits quality engineers and managers, operations and production leaders, EHS professionals, Six Sigma practitioners and anyone who investigates defects, incidents or failures.

Root cause analysis (RCA) training is a practical skills course that teaches you to investigate problems and eliminate their underlying causes. It suits quality engineers and managers, operations and production leaders, EHS professionals, Six Sigma practitioners and anyone who investigates defects, incidents or failures.

Yes. Ireland's medtech, pharma and manufacturing sectors depend on rigorous problem-solving and CAPA, and structured RCA skills are in demand across quality, operations and improvement roles. The method applies immediately at work, helping reduce repeat defects and strengthen audit-ready corrective actions.

No. There are no formal prerequisites. The programme introduces RCA principles and tools from first principles, so it suits both newcomers and experienced professionals. Some familiarity with quality or process improvement is helpful but not required.

The course covers a full toolkit: the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in an RCA context, Is/Is Not analysis and the 8D methodology. You also learn how to select the right tool based on a problem's complexity and severity.

RCA is the investigative backbone of ISO 9001:2015 corrective action (Clause 10.2) and of CAPA processes required by FDA, EU MDR and ISO 13485. The training equips you to determine causes, act to prevent recurrence and document it in a way regulators and auditors expect.

    General

What is root cause analysis training and who is it for?

Root cause analysis (RCA) training is a practical skills course that teaches you to investigate problems and eliminate their underlying causes. It suits quality engineers and managers, operations and production leaders, EHS professionals, Six Sigma practitioners and anyone who investigates defects, incidents or failures.

Is RCA training worth it for professionals in Ireland?

Do I need prior experience to take the course?

No. There are no formal prerequisites. The programme introduces RCA principles and tools from first principles, so it suits both newcomers and experienced professionals. Some familiarity with quality or process improvement is helpful but not required.

Which RCA tools does the course cover?

The course covers a full toolkit: the 5 Whys, the Fishbone (Ishikawa) diagram, Fault Tree Analysis, FMEA in an RCA context, Is/Is Not analysis and the 8D methodology. You also learn how to select the right tool based on a problem's complexity and severity.

How does RCA relate to ISO 9001 and CAPA?

RCA is the investigative backbone of ISO 9001:2015 corrective action (Clause 10.2) and of CAPA processes required by FDA, EU MDR and ISO 13485. The training equips you to determine causes, act to prevent recurrence and document it in a way regulators and auditors expect.

    Pricing

How much does root cause analysis training cost in Ireland?

Fees vary by delivery mode, group size and format, so pricing is available on request. Individual and corporate group options are offered. Request a quote and a learning advisor will share current pricing for your preferred format.

What is included in the course fee?

The fee typically includes instructor-led training across all six RCA modules, hands-on exercises and case studies, learning materials, and a course completion certificate from Invensis Learning. Ask about the exact inclusions for your chosen format.

Do you offer group or corporate rates?

Yes. We offer corporate group training with schedules and content tailored to your team, plus volume-based pricing for larger groups. Contact us for a tailored quote for your quality, operations or EHS team.

Are there discounts for booking a full team?

Group bookings are our most cost-effective option, and using your own live problems as case studies adds immediate value. Talk to a learning advisor about the best package for the number of people you want to train.

What payment options do you offer?

We accept major cards and bank transfers, and instalment options are available on selected bookings. For corporate training we can raise an invoice against a purchase order. Request a quote and an advisor will set up the option that suits you.

    Assessment

Is there an exam for the RCA course?

No. This is a non-certification skills course with no examination. Learning is reinforced through hands-on RCA exercises, case study investigations and team discussion during the programme.

How is learning assessed during the training?

There is no formal assessment. Instead, you apply each tool through practical exercises, such as an Is/Is Not problem definition, a 5 Whys investigation, a Fishbone workshop and an 8D report, so you build capability by doing rather than by sitting a test.

Do I receive anything on completion?

Yes. On completing the programme you receive a course completion certificate from Invensis Learning. There is no external credential or awarding body, as this is a practical skills course rather than a certification.

Will I be able to apply the tools straight away?

Yes. The course is built around workplace application. By the end you can define a problem precisely, run 5 Whys and Fishbone sessions, apply FTA and 8D, and develop and verify corrective actions, all skills you can use the next working day.

Does the course lead to an external credential?

No. RCA is a vendor-neutral skills discipline with no single awarding body, so the course does not lead to an external credential. It builds practical capability grounded in recognised quality frameworks such as ISO 9001 corrective action and the 8D methodology.

    Course Content

What topics are covered in the RCA training?

The programme covers six modules: RCA principles and problem types, problem definition and data collection, the 5 Whys, the Fishbone diagram and advanced tools such as FTA and FMEA, the 8D methodology, and corrective action, verification and building an RCA culture.

    How long is the course?

The training is delivered as a one-day, eight-hour programme covering all six RCA modules with concept explanations, worked examples and hands-on exercises throughout. It can also be arranged as two half-day sessions for corporate groups.

Is the course relevant to non-manufacturing sectors?

Yes. While RCA has deep manufacturing roots, the tools apply across healthcare, IT service management, financial services, energy and the public sector. The course shows how to adapt cause categories and tool selection to your own context.

Does the course include hands-on practice?

Yes. Hands-on RCA exercises and case study investigations run throughout the day, including 5 Whys, Fishbone workshops and completing an 8D report for a realistic scenario, so you leave with practical, applied experience.

What materials are provided with the training?

You receive structured learning materials covering all six modules, worked examples for each RCA tool, and templates and reference guides you can reuse at work, such as Fishbone and 8D formats. Materials support both the sessions and your later investigations.

    Enrollment

How do I enrol in the RCA course?

You can enrol through the Invensis Learning website. Choose your preferred format, complete your booking, and you will receive access to schedules, materials and joining details. Talk to a learning advisor if you need help choosing.

    What delivery formats are available?

The course is available as online (live virtual) instructor-led training, in-person classroom training, and customised corporate on-site training, so you can choose the format that fits your schedule and team.

    How soon can I start?

Public sessions run regularly, and corporate groups can arrange dates to suit team availability. See upcoming schedules or request a quote, and an advisor will confirm the next available options for you.

Can training be delivered at our own site in Ireland?

Yes. Corporate on-site delivery is available across Ireland, and sessions can use your organisation's real problems as case studies for immediate value. Contact us to arrange on-site or live virtual training for your team.

What is the reschedule or cancellation policy?

Rescheduling and cancellations are handled per our published policy, and flexible options are available on selected bookings. Reach out to our team for the details that apply to your enrolment.
